Position Overview:

We are seeking a Clinical Pharmacist who is passionate about providing patient-centered care and making a direct impact on clinical outcomes. This full-time position offers the opportunity to work in a dynamic, collaborative environment while utilizing your expertise in pharmacology to support patient care across various clinical specialties.

Key Responsibilities:

  • Medication Management : Review and assess medication orders for appropriateness, safety, and efficacy, providing recommendations for therapeutic alternatives when necessary.
  • Clinical Collaboration : Work closely with physicians, nurses, and other healthcare professionals to ensure optimal drug therapy and outcomes.
  • Patient Education : Provide counseling to patients on proper medication use, side effects, and interactions to ensure adherence to prescribed therapies.
  • Pharmacokinetic Monitoring : Adjust medication regimens based on individual patient factors such as kidney and liver function, age, and weight.
  • Drug Information : Serve as a resource for up-to-date drug information and best practices for drug therapy management.
  • Education & Training : Mentor and educate pharmacy residents, students, and other healthcare team members on pharmacological therapies.

Qualifications:

  • Education : Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
  • Licensure : Active Pharmacist license in the state of Indiana, or eligibility for licensure.
  • Experience : Previous experience as a clinical pharmacist is preferred, but new graduates with a strong clinical background are encouraged to apply.
  • Certifications : Board certification in a clinical pharmacy specialty (e.g., BCPS) is a plus, but not required.
  • Skills : Strong clinical judgment, excellent communication skills, and the ability to work effectively in a team-oriented environment.
  • Knowledge : In-depth knowledge of pharmacology, therapeutic protocols, and medication management practices.
