Clinical Rehab Nurse Manager - Full-Time 1st Shift

Full Time Role

Starting Pay: 85,000 to 90,000 (Salaried role and pay is based on previous experience)

5 days per week schedule

On‑call manager rotation

Position Purpose

The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of a coordinator with assessing, planning, implementation, monitoring and evaluation of the options and services required to meet the residents’ health service needs.

Primary Job Duties

• Clinical Rehab Nurse Manager participates in welcoming new residents and their families to the community.
• RN initiates and supervises the completion of Physician Certification & recertification for rehab stay.
• Manages resident and family concerns in a calm & professional manner.
• Serves as a nursing representative for PAN, Bundle & Medicare meetings.
• Participates in resident baseline and comprehensive care plan for continuity of care.
• Preparation, review and submission of insurance documentation, authorization, appeals and audits.
• Assessing, implementing, and evaluating resident individual level of care needs and ensuring that is reflected in resident care plan.
• Monitors daily Nursing documentation to ensure resident condition and treatment is being appropriately documented.
• Participates in the Nurse on Call rotation.
• Other duties as assigned.

Requirements

• Registered Nurse RN license in good standing
• 3+ years clinical experience
• Minimum 1 year experience in long term care.
