Position: The Clinical Pharmacy Specialist – Solid Organ Transplant is a decentralized clinical position that supports WVU Medicine’s kidney and heart transplant programs, and the development of the liver transplant program. The pharmacist will provide comprehensive pharmaceutical care to adult solid organ transplant recipients across the continuum of care, including pre-transplant evaluation, peri-operative management, inpatient monitoring, discharge coordination, and outpatient transplant clinic follow-up. The pharmacist will serve as the pharmacology expert on the interdisciplinary transplant team, optimize complex immunosuppressive and prophylactic regimens, manage high-risk drug interactions, and ensure safe and effective medication use in a high-acuity, highly regulated environment.

This is a Monday–Friday day shift role with no weekend, holiday, or on-call responsibilities, and is fully on-site. The role also includes precepting pharmacy students and residents, contributing to transplant program development and workflow design (especially for liver transplant services), participating in interdisciplinary committees, and engaging in research, quality improvement, and scholarly activities. Strong clinical expertise in transplant pharmacotherapy, excellent communication and collaboration skills, and the ability to support program growth while maintaining regulatory compliance are essential for success.

Responsibilities

• Monitor drug therapy by participating in medical rounds or evaluating the patient’s medication regimen, medical problems, laboratory data, and special procedures.
• Provide leadership and care for patients in assigned areas; guide providers and patients on best practices and ensure safe and effective drug use through clinical involvement.
• Assist in admission and discharge planning and patient education related to the appropriate use of medication.
• Provide drug information and educational programs to medical and nursing staff, other health care practitioners, and pharmacy staff.
• Maintain professional competence by attending/presenting at conferences and continuing education, and by reading current literature.
• Participate in administrative, quality assurance, performance improvement, and clinical research activities within the department; participate in medication use evaluations.
• Review each order to verify accuracy and appropriateness; evaluate compliance with medication safety procedures; suggest improvements (e.g., ISMP).
• Document medication errors, unsafe conditions, or near misses; promote a constant focus on medication safety.
• Dispense medications per provider orders and ensure policies related to drug distribution are followed; oversee drug use control in assigned areas.
• Identify problems related to drug use control and contribute to resolution; make decisions regarding pharmaceutical services in the assigned service area.
• Detect, report, and help minimize adverse drug reactions and medication errors; participate in management of acute/chronic disease states and medical emergencies.
• Communicate findings and recommendations, and document in the clinical information system; maintain required records per legal and hospital policy.
• Supervise technicians in drug preparation and distribution; provide objective feedback to co-workers to support performance management.
• Participate in orientation/education of pharmacy students, residents, and new department staff; engage in educational and scholarly activities related to drug therapy.
• Assist in planning and implementing changes to improve pharmacy productivity; complete timely written assignments and continuing education documentation; complete annual clinical competency evaluations.

Qualifications

• Must possess current licensure as required by the state board where services will be provided: WV – Pharmacist license (West Virginia State Board of Pharmacy); PA – Pharmacist license (Pennsylvania State Board of Pharmacy); MD – Pharmacist license (Maryland Board of Pharmacy); OH – Pharmacist license (Ohio Board of Pharmacy).
• Obtain certification in Basic Life Support within 60 days of hire date.

• Doctor of Pharmacy (Pharm.D) from an ACPE-accredited school of pharmacy.
• Experience in at least one of the following: completion of a PGY-2 Residency in an applicable clinical field; or PGY-1 Residency with Board Certification; or PGY-1 Residency with three years of relevant pharmacist experience; or three years of relevant pharmacist experience with Board Certification.

Physical Requirements

• Walk and stand for long periods; ability to lift up to 30 pounds and push up to 50 pounds; dexterity to finger, handle, and reach.
