We’re hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands‑on role in elevating patient care at a nationally recognized Academic Medical Center. This dynamic, patient‑centered position involves building meaningful relationships with patients and providers while delivering high‑quality medication management on site in clinic and via virtual patient assessments under a Collaborative Practice Agreement (CPA).

Salary
Employees who perform this role in Washington can expect a salary rate between $130,000 and $150,000 (hourly equivalent: approximately $62.50 – $72.12 per hour, based on a 40‑hour workweek). Bonus compensation is governed by applicable bonus plans.

Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Monitor therapy effectiveness and safety, identify optimization needs, and intervene as appropriate.
- Support quality‑improvement and REMS initiatives.
- Provide recommendations to operations by supporting teammates with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
Qualifications
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Candidate must meet one of the following qualifications:
- Completion of a PGY2 Ambulatory Care Residency
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a CPA
- At least 3 years of ambulatory practice with disease state management
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team‑first mindset
