Mountain Community Health Partnership (MCHP) is hiring a full-time Clinical Pharmacist to enhance patient care as part of a multidisciplinary team. Key responsibilities include:
- Providing direct patient care and consulting with providers on medication access and management of high-risk patients.
- Managing chronic diseases through collaborative practice agreements and patient education.
- Evaluating medication appropriateness and assisting with the 340B program.
- Administering and monitoring long-acting injectables.
Applicants must hold a PharmD, be eligible for North Carolina CPP, and preferably have PGY1/PGY2 residency or 1-2 years of ambulatory care experience. An active North Carolina license is required. MCHP values dignity, teamwork, integrity, compassion, quality, and equity.
