A large hospital within the Texas Medical Center is seeking an Oncology Inpatient Pharmacist for a contract position based in Spring, TX. This role is integral to the patient care team, focusing on medication management for oncology patients in a busy 500+ bed facility.
Responsibilities include:
- Rounding with the multidisciplinary patient care team to assess and optimize medication therapy
- Providing clinical recommendations on medication interventions
- Designing and updating treatment protocols in Epic and Epic Beacon systems
- Facilitating smooth transitions of care between inpatient and outpatient oncology pharmacy services
- Submitting non-formulary medication requests as needed
- Coordinating the procurement and administration of specialty medications for oncology patients
Qualifications:
- Active Texas pharmacist license
- Texas IV certification
- Minimum 3 years of inpatient pharmacy experience
- Oncology pharmacy experience preferred
- Experience working with Epic electronic medical record system highly desirable
