The University of Iowa Healthcare Department of Pharmaceutical Care is looking for full-time Clinical Pharmacists to join our team.

Overview

The Specialty Pharmacy and Ambulatory Care Clinical Pharmacist provides pharmaceutical care to patients within the UI Specialty Pharmacy Program and patients seen in UI Healthcare Ambulatory Care Pharmacies. The pharmacist delivers clinical support, education, dispensing, and monitoring of patients receiving medications through UI Healthcare Ambulatory Care Pharmacies, including Specialty Pharmacy. Services are provided through face-to-face, mail order, and telehealth. Adequate training to Specialty Pharmacy and Ambulatory Care Services is provided. The pharmacist serves as a role model for other Department of Pharmaceutical Care staff, residents, and students, particularly in the provision and expansion of ambulatory pharmacy services. The role rotates between offsite central fill pharmacy and office locations in Iowa City and supports both adult and pediatric ambulatory care operations at UI Health Care Medical Campus.

Selective candidates are eligible for a sign‑on bonus up to $10,000.

Position Duties

• Provides Therapeutic Drug Monitoring (TDM) on clinic‑administered medications and take‑home prescription orders.
• Completes a comprehensive patient assessment, including:
  • Evaluation of the patient’s medications and medical history.
  • Assessment of all prescription orders for appropriateness in indication, dose, route, drug interactions, dose adjustments for organ dysfunction, physical and therapeutic incompatibilities, drug‑induced side effects and adverse reactions, potential drug misuse or abuse, cost‑effective therapy, and duplicate therapy.
• Ensures pharmacokinetic assessment of drug dosing on targeted patients through clinical assessment and preparation of formal dosing recommendations.
• Communicates with prescribers about patient medication needs.
• Conducts review of patient profiles and updates them as needed based upon findings at the time of patient visit.
• Provides appropriate drug information to other healthcare professionals.
• Confirms and clarifies all patient drug allergies noted in the medical record.
• Participates in collecting and documenting medication histories on patients.
• Evaluates patient compliance with prescribed medication therapy and identifies problems that may lead to partial or non‑compliance.
• Utilizes appropriate resources and coordinates the resolution of financial concerns with department financial counselors.

Documentation of Patient Care Activities

• Patient drug allergies and patient‑specific height and weight information.
• Medication histories.
• Medication counseling to patients.
• Interventions with prescribers and other healthcare providers.
• Patient interactions in the clinic and elsewhere (home, inpatient, at Ambulatory Care Pharmacy).
• Drug dosing recommendations made to prescribers.
• Drug Use Evaluation Program activities.
• Steps taken to identify, evaluate, or avoid adverse drug reactions.

Patient Communications and Interactions

• Educates patients on the pharmaceutical care services offered, including medication history, drug allergy history, and medication counseling.
• Provides patient teaching classes (where appropriate).
• Communicates patient information to local healthcare providers involved with the patient’s care to ensure continuity of care.

Operational Initiatives

• Follows up on medication system problems such as unclear or inappropriate prescription orders, cost‑effective drug therapy, and prescription for non‑formulary products.
• Carries out Medication Use Evaluation Program initiatives through active prescriber intervention and education.
• Serves on inter‑departmental committees, task forces, and teams as required.
• Facilitates completion of monthly clinic audits as assigned by the supervisor.

Educational Services

• Serves as preceptor for pharmacy students and pharmacy residents.
• Presents in‑service educational programs to staff nurses, physicians, and other healthcare team members.
• Participates in educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UI Health Care or UI departments, and state and national organizations.
• Conducts patient education classes about the proper use of medications.
• Mentors and serves as a role model for other pharmacist practitioners within the department.

Transitions of Care

• Participates in the facilitation of patient transfer from the inpatient to the outpatient setting, including bedside delivery of discharge medications directly to the patient for education and expediting discharge.

Research

• May participate in research activities, including support for Investigational Drug Service prescriptions and orders.
• Encouraged to present and/or publish research findings.

Qualifications

• Education: PharmD (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy.
• Experience:
  • One (1) year of applicable pharmacy experience required; prior work or student rotational experience may be considered relevant.
  • Immunization certification.
  • Current licensure by the Iowa Board of Pharmacy Examiners (or eligibility for licensure).
  • Excellent written and verbal communication skills.
  • Professional experience working effectively with individuals from diverse backgrounds.

Desired Qualifications

• Completion of an ASHP accredited PGY1 Residency.
• Health system pharmacy experience.
• Specialty Pharmacy experience.
• Clinic order experience.
• Non‑sterile compounding experience.
• Board certification or certification in Ambulatory Care, Pharmacotherapy, Oncology, Specialty Pharmacy or other pertinent area.
