In this collaborative, patient-centered role, you will optimize medication therapy, perform order verification, therapeutic drug monitoring, and medication reconciliation, and round with interdisciplinary teams. You will support evidence-based protocols, educate staff and patients, and promote medication safety while floating to areas of highest need across our integrated health system.
