## CLINICAL PHARMACISTApplyremote type: Onsitelocations: Samaritan Medical Centertime type: Casualposted on: Posted Todayjob requisition id: R3242Location:Samaritan Medical CenterDepartment: SMC PHARMACYPay Range:$55.27 - $91.19Care for our community, and your career.**Key Responsibilities** Medication Management and Clinical Review* Maintain and update patient profiles, including demographics, diagnoses, allergies, and* current medications.* Dispense medications accurately and promptly in accordance with prescription orders and* established protocols.* Evaluate medication therapy, addressing potential problems and clarifying orders with* prescribers when necessary.* Review drug regimens for appropriateness, considering indication, health status,* therapeutic goals, allergies, and adverse drug reactions.* Perform renal dose adjustments, IV to PO conversions, and optimize antibiotic therapy as* needed.* Monitor for and manage drug-drug, drug-food, and drug-disease interactions, dosage* issues, contraindications, and duplication of therapies.* Conduct follow-up evaluations to assess therapy effectiveness and address evolving* medication needs.* Help respond to drug shortages* Reviews of policies and procedures* Pharmacokinetic dosing of vancomycin and aminoglycosides* Stays abreast of current clinical guidelines **Patient Care and Interdisciplinary Collaboration*** Participate in daily patient care review meetings and multidisciplinary care teams as* assigned.* Responsible for reviewing and verifying the accuracy and completeness of medication* histories obtained by pharmacy technicians.* Provide drug information and recommendations to physicians, nurses, and other* healthcare professionals.* Support emergency medical care by participating in drug therapy management during* medical emergencies.* Education and Oversight* Precept pharmacy students during clinical rotations.* Prepare educational materials, including newsletters, memos, and presentations on drug related topics for staff. **Regulatory Compliance and Quality Assurance*** Ensure adherence to facility and pharmacy policies, medication orders policy, and* clinical guidelines.* Maintain compliance with all applicable regulatory bodies (e.g., Board of Pharmacy,* CMS, Joint Commission).* Monitor and report adverse drug reactions and medication errors.* Participate in target drug programs and drug utilization reviews, reporting findings to the* Pharmacy and Therapeutics Committee as requested.* Operational Duties* Oversee the preparation, labeling, and verification of pharmaceutical products, including* oral, IV, and compounded medications.* Check unit dose medication batches and maintain accuracy in Omnicell, Max trays etc.* Ensure secure handling and accountability of controlled substances.* Perform drug selection and procurement activities based on safety, efficacy, and cost effectiveness.* Practice flexibly across different areas of the pharmacy as workload demands. **Supervisory Responsibilities*** By profession and licensure, supervises pharmacy technicians (assistants), buyers, and* pharmacy interns.* Accountable for all delegated tasks, ensuring that supportive personnel perform duties in* compliance with regulations and standards.* Maintains awareness of all compounding, preparation, packaging, labeling, and* dispensing activities.* Ensures proper record-keeping and adherence to all pharmacy procedures**Work Shift:**FLSA7DAY- 8 Hours Night Shift (United States of America)
