Schedule: flexibility to work rotating shifts between the hours of 8:00am and 8:00pm CST, Monday through Friday. Extended hours and occasional weekends may be required as needed.
About the Role
The Clinical Pharmacist plays a key role in delivering telephonic consultations to members, focusing on comprehensive medication reviews, and improving disease state management.
Key Responsibilities
- Conduct telephonic consultations and schedule appointments for members enrolled in clinical programs.
- Utilize prescription claims data to provide clinical recommendations under MTM and Retrospective Drug Utilization Review protocols.
- Identify gaps in care, high-risk medications, compliance/adherence issues, drug interactions, and opportunities for therapeutic or generic substitution.
- Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions.
- Educate patients on medication regimens and disease states.
- Document patient care notes and recommendations accurately and professionally.
- Stay current with emerging clinical trends and participate in departmental meetings.
- Meet productivity and performance expectations.
- Support special projects and other duties as assigned.
Base Pay Range
$65.00/hr - $65.00/hr
