To support a Clinical Engagement Services team, the remote Utah Licensed Clinical Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and provide education on medication and disease states. Key responsibilities Conduct telephonic consultations and comprehensive medication reviews Identify gaps in care, adherence concerns, and drug interactions Document clinical interventions and communicate recommendations to patients and healthcare providers Required qualifications Current, unrestricted Pharmacist license in Utah Strong knowledge of Medication Therapy Management (MTM) and disease-state management 1+ year of remote or independent work experience PharmD and PBM/retail experience preferred Bilingual skills are a plus
