Seamless Assist is seeking a full-time, remote clinical pharmacist with cardiology expertise to support value-based care programs. The focus is GDMT optimization, titration, and close collaboration with RN Care Managers and cardiology providers to close therapy gaps.
This is a non-dispensing, telephonic role embedded in the care team. You will document recommendations in the care management platform/EMR, educate patients, and adhere to Central Time business hours. Texas license required.
