Remote Patient Care Pharmacist – Virtual Counseling & Adherence Support | Work From Home

Key Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.

What You’ll Bring

• Education: PharmD or Bachelor of Pharmacy.
• Licensure: Active U.S. pharmacist license (multiple state licenses preferred but not required).
• Experience: Ambulatory care, MTM, or clinical preferred. Hospital and community pharmacists passionate about patient interaction are encouraged to apply.
• Skills: Strong communication, empathy, and ability to build rapport virtually.
