Job Summary and Qualifications

Position Summary: Identifies, resolves, and prevents drug-related problems. Recommends and monitors patient drug therapy and provides drug information. Interacts with patients, family, and other healthcare providers to assure continuity of services and achieve desired patient outcomes through safe, timely, cost-effective drug therapy. Participates in professional development, training of students and residents, and clinical research.

Job Specific Duties

  • Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
  • Interprets medication orders (verbal and written) and transcribes them to computerized patient medication profiles accurately.
  • Maintains accurate, complete patient medication profiles.
  • Calculates chemotherapy, parenteral nutrition, pediatric doses, and continuous infusions accurately and performs required double-checks on chemotherapy and pediatric dosing.
  • Accurately compounds and dispenses medications; labeling follows departmental procedures; products are bar-coded according to departmental procedures; identifies and selects specific dosage forms appropriate for all ages of patients served; assures compliance with organizational policy regarding home medication, self-administration, and bedside storage of medications.
  • Handles controlled substances: resolves discrepancies and notifies pharmacy and hospital administration of unresolved discrepancies or suspected diversion; issues controlled substances to patient care areas and maintains records as required by law; receives schedule II controlled drugs correctly and oversees pharmacy technicians receiving all controlled substances.
  • Prepares sterile products: chemotherapy/hazardous drugs, parenteral nutrition (TPN), small and large volume parenterals, and other sterile products such as eye drops and irrigation solutions; cleans, operates, and maintains the laminar airflow and hoods and biological safety cabinet according to departmental procedures; ensures routine cleaning of room and surfaces; wears appropriate garb and washes hands; demonstrates aseptic technique and passes media fill testing; disposes of sharps and hazardous waste; accurately dispenses and labels sterile products.
  • Manages crash carts and emergency medications: ensures refill procedures are accurate and completed before shift end and as needed; assures new crash carts are delivered within 15 minutes of code notification; documents required procedures accurately.

Qualifications

  • Pharmacist license required.
  • Bachelor’s, Master’s, or Doctorate degree in Pharmacy.
  • No travel required.
