Summary
UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated pharmacist interested in joining the UNC Adult Neurology Clinic.
The successful Clinical Pharmacist Practitioner (CPP) will play an integral role in the care of patients within the Neurology Division, supporting the Rapid Access Neuroimmunology and Neuromuscular program. This is a multidisciplinary ambulatory clinic focused on providing timely, coordinated care for patients with complex neurological conditions requiring expedited evaluation and treatment.
Role Overview
The Neurology CPP will serve as an Advanced Practice Provider on the interdisciplinary health care team and provide clinical support to neurologists, nurses, and ancillary staff. The CPP will support rapid access to care for patients presenting with new or worsening neuroimmunologic or neuromuscular disease, with a focus on avoiding unnecessary emergency department visits and hospital admissions.
Core Responsibilities
- Conduct pharmacotherapy visits, manage high‑cost and complex medication regimens, and coordinate initiation of specialty therapies (infused and specialty medications).
- Optimize treatment plans, and provide longitudinal medication monitoring and follow‑up.
- Coordinate care transitions between inpatient, outpatient, and infusion settings, collaborating closely with specialty pharmacy, infusion services, and care coordination teams.
- Provide educational mentorship and experiential precepting to pharmacy students, PGY1 and PGY2 Pharmacy residents, and neurology trainees.
- Partner with the Department of Pharmacy and Neurology leadership to support institutional and departmental quality improvement initiatives focused on access, care coordination, and clinical outcomes.
- Academic appointment at the UNC Eshelman School of Pharmacy is available commensurate with experience.
Qualifications
Completion of a PGY2 specialty residency in Ambulatory Care, Pharmacotherapy, or a related specialty (e.g., Neurology) or an equivalent level of clinical experience is preferred. Three to five years of ambulatory care or specialty practice experience is strongly preferred. Board certification in Ambulatory Care, Pharmacotherapy, or a relevant specialty and prior experience as a resident or student preceptor are also preferred.
Education Requirements
- Bachelor's degree in Pharmacy.
Licensure/Certification Requirements
- CPP must be a registered Pharmacist and licensed CPP in the state of NC.
- Credentialing and Privileging must be completed through the appropriate UNC HCS accrediting body as an APP prior to start date.
Professional Experience Requirements
- North Carolina registered Pharmacist: BS Pharmacy or Pharm D, BPS Certification or Geriatric Certification or ASHP residency program + 2 years of experience.
- North Carolina Registered Pharmacist: Pharm D, 3 years experience, 1 certificate program.
- North Carolina Registered Pharmacist: BS Pharmacy, 5 years experience, 2 certificate programs.
