Clinical Pharmacist PD - Inpatient

Summary

Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor, is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel. Responsibilities

• Receive patient medication orders as written by a physician or practitioner and review/interpret each order for completeness and therapeutic appropriateness, considering patient clinical status, concomitant drug therapy, diet, and developmental stage.
• Contact prescriber or covering prescriber as necessary to initiate interventions that ensure therapeutic efficacy, minimize risk, and optimize cost effectiveness.
• Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective medication use for all patient populations served.
• Provide drug information and consultation to prescribers, nurses, and other health‑care professionals regarding appropriate drug therapy, including interactions, side‑effects, incompatibilities, allergies, order discrepancies, stability problems, and administration concerns.
• Proactively review and approve medication orders in LifeChart, ensuring dose, dose form, frequency, and duration are appropriate and that relevant patient parameters (diagnosis, allergies, weight, age, etc.) are present.
• Supervise pharmacy interns and technicians to maintain performance standards and compliance with federal, state, and organizational regulations.
• Identify, document, resolve, monitor, and prevent drug‑therapy problems such as contraindications, duplications, and drug interactions.
• Engage with other clinical pharmacists or specialist pharmacists for complex clinical situations.
• Integrate patient‑focused medication therapy management and pharmaceutical care according to established clinical pathways or guidelines.
• Participate in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications, recommending dose adjustments and laboratory monitoring as required.
• Ensure compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs.
• Document and report all clinical interventions, medication errors, near misses, and adverse drug events per established protocols.
• Participate in emergency code blue response.
• Attend and participate in daily medical, multidisciplinary, and specialty rounds as time allows.
• Review and approve formulations for sterile products requiring compounding or manipulation prior to dispensing, completing in‑process and final checks for medications prepared by pharmacy technicians.
• Participate in education and training of health‑care practitioners, including residents, students, interns, technicians, physicians, and nurses on medication use matters.
• Serve as preceptor to pharmacy residents and students.
• Participate in continuous quality improvement activities and total quality management initiatives, including adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs.
• Assist in coordination of transitions of care, as appropriate.
• Maintain professional competence through participation in pharmacy staff development and skills enhancement programs, independent study, memberships, and professional organization participation.

Qualifications

• Basic Knowledge: Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy.
• Licensure: Current unrestricted licensure as a licensed pharmacist or a 90‑day temporary pharmacist in Rhode Island at start date; obtain full licensure before the expiration of the temporary license and maintain unrestricted licensure thereafter.
• Certifications: American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications within 120 days of hire.
• Experience: Successful completion of Post Graduate Year 1 (PGY‑1) residency or national board certification in pharmacotherapy preferred; PGY‑2 residency in a specialty area strongly preferred if applicable. Clinical experience may be considered in lieu of formal training.
• Skills: Effective interpersonal and communication skills, both written and verbal; effective organizational and presentation skills.
• Technical: Knowledge of area/specialty‑specific technology, including pharmacy information systems, automated dispensing technology, software, equipment, and automation.
• Computer: Experience with MS Word, Excel, Outlook, and web browsers.

Work Environment and Physical Requirements

Extended periods of standing and walking; requires visual acuity and manual dexterity to operate a computer. Occasional travel between facilities or sites as required.

Independent Action

Functions independently within the broad scope of departmental policies and practices; refers specific complex problems to supervisor when clarification of departmental policies and procedures may be required.

Pay Range

Pay Range: $50.49 - $83.30 per hour
