The salary range for this position is $60.10 – $78.13 per hour. Placement within the range is based on relevant work experience, internal equity, and collective bargaining agreement rules for union‑represented positions.

Description

The Clinical Pharmacist reflects the mission, vision, and values of the organization, adheres to the Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory and accreditation standards.

The Clinical Pharmacist is responsible for identifying, preventing, and/or resolving medication‑related problems; providing authoritative information to healthcare professionals, patients, and the public; and ensuring that the informational, clinical, and drug distribution components of pharmacy services function as an integrated system to provide patient‑centered care.

Weekday Hours

7:30 am – 4:00 pm or 11:30 am – 8:00 pm

Weekend Hours

7:30 am – 4:00 pm (every third weekend)

Responsibilities

• Interpret medication orders to ensure safety and proper use, monitor compatibilities, incompatibilities, medication duplication, allergies, and potential administration errors.
• Monitor medication therapy for significant drug‑drug, drug‑diet, or drug‑laboratory test interactions and intervene when necessary.
• Evaluate and document that selected medications are appropriate for achieving therapeutic outcomes and communicate potential problems and solutions to prescribers.
• Consult on pharmacokinetics and adverse reactions to physicians, nurses, and other healthcare professionals; participate in therapeutic monitoring programs and adjust therapy as indicated.
• Process all medication order information into the Electronic Health Record to ensure accuracy.
• Maintain a comprehensive pharmacy patient profile including age, weight, allergies, diagnosis, and pertinent clinical parameters.
• Oversee packaging, labeling, and distribution of medications for patients and employees following established policies and controlled inventory procedures.
• Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
• Verify medication orders before delivery to patient care areas and resolve discrepancies.
• Review laboratory data for medication appropriateness and recommend changes when needed.
• Develop and present orientation, education, and training programs for staff and assist in educating pharmacy students, residents, and fellows.
• Educate patients and families about medications.
• Respond to emergency situations to assist with medication management (e.g., cardiac arrest).
• Provide accurate and timely medication information and education to professionals or patients.
• Maintain the hospital formulary and limit medication usage to formulary items when appropriate.
• Document all clinical activities and interventions, collect data, and maintain logs as required by the department.
• Communicate professional knowledge effectively in oral and written forms to patients and other professionals.
• Participate in quality improvement and medication use review activities.
• Organize, prioritize, and plan workload within given time constraints; cooperate with fellow pharmacists to oversee and direct work area activities, assign tasks to technicians, and achieve department objectives.
• Review restocking of automated dispensing cabinets throughout the hospital and resolve any problems.
• Ensure compliance with hazardous waste collection and disposal procedures.
• Maintain proficiency and expertise in areas of practice through continuous professional development.
• Perform all other job duties as assigned.

Qualifications

Required:

• Current license to practice pharmacy in the State of Illinois.
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy (Pharm.D.) Degree.

Preferred:

• Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.).
• Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
