To support innovative pharmacy care, the full-time Clinical Pharmacist - Multi-State Licensed (Bilingual - Cantonese/Mandarin) will manage medication therapy, collaborate with healthcare providers, and educate patients on chronic disease management while working remotely during Pacific or Central hours. Key responsibilities Conduct comprehensive medication reviews and develop personalized care plans for patients managing chronic conditions Collaborate with interdisciplinary teams to enhance disease management processes and ensure medication adherence Drive performance on pharmacy-related clinical measures and mentor the wider team on pharmacotherapy topics Required qualifications Active pharmacist licensure in good standing in at least five states, including Alabama, Florida, or Texas Minimum of three years of clinical experience in direct patient care Fluent proficiency in Cantonese and Mandarin Ability to work during typical clinic hours in Mountain, Pacific, or Hawaiian time zones Experience with collaborative pharmacy practice agreements preferred
