Clinical Pharmacist - Internal Medicine/Critical Care

The Department of Pharmacy at Mount Auburn Hospital is seeking a Clinical Pharmacist for Internal Medicine and Critical Care. This pharmacist will work in a decentralized role, rounding with Internal Medicine, Intermediate Care, or Critical Care teams, providing clinical coverage for medical and surgical patients. They will operate predominantly on a Monday‑Friday day shift schedule, with rotating weekends, holidays, and evening/overnight coverage as needed.

Qualified candidates are eligible for a sign‑on bonus.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Practice in compliance with all applicable state and federal laws, regulations, and accrediting bodies.
• Adhere to and interpret institutional, departmental, and pharmacy policies and procedures.
• Maintain required licensure, certifications, credentials, and continuing education.
• Assume operational responsibility for assigned pharmacy service areas and shift activities without direct supervision.
• Provide staffing coverage across pharmacy areas as trained and assigned.
• Uphold a culture of professionalism, respect, teamwork, and patient safety within the Pharmacy Department.
• Coordinate and supervise the work of pharmacists, technicians, students, residents, interns, and other supportive personnel.
• Participate in onboarding, orientation, training, mentorship, precepting, and performance evaluation of pharmacy staff and learners.
• Prepare, compound, verify, and dispense medications (including sterile products, unit dose, bulk, and extemporaneous compounds) accurately and safely.
• Ensure the accuracy, sterility, and timeliness of all medication preparations and intravenous products.
• Maintain accurate records and controls for controlled substances, investigational drugs, and clinical research medications.
• Review, interpret, and verify medication orders for appropriateness, accuracy, and patient‑specific factors; clarify discrepancies with prescribers.
• Provide prospective evaluation, monitoring, and optimization of drug therapy, including pharmacokinetic dosing and surveillance activities.
• Identify, document, resolve, and help prevent medication‑related problems, adverse drug events, and near misses.
• Document and communicate clinical interventions, consults, and medication‑related information to ensure continuity of care.
• Provide drug information and clinical consultation to physicians, nurses, and other healthcare professionals.
• Support transitions of care through medication reconciliation, discharge counseling, and preparation/dispensing of ambulatory prescriptions.
• Participate in quality assurance, utilization review, drug use evaluations, and other departmental or institutional initiatives.
• Perform other duties as assigned.

QUALIFICATIONS

• Bachelor of Science in Pharmacy (five‑year degree program) from an accredited college of pharmacy.
• Licensure to practice in the Commonwealth of Massachusetts from the Board of Registration in Pharmacy.
• Good oral and interpersonal communication skills to effectively interact with healthcare disciplines and pharmacy staff.
• Extensive knowledge and ability in all areas of the department, including IV admixtures, drug information, ambulatory care, manufacturing, and drug distribution.
• Comprehensive knowledge of pharmacotherapy.
• Ability to work collaboratively as a member of the patient care team.

• Be able to stand or walk for prolonged periods of time (approximately 70% of the time) and lift up to 25 lbs.
• Be aware of occasional exposure to infectious disease and to possible carcinogenic, mutagenic, and sensitizing medications; minimal exposure to caustic agents, acids, and fumes.

PAY RANGE

$120,640.00 USD – $162,364.80 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, including seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
