Job Title: Clinical Pharmacist - Inpatient
Location: Mayfield Hts
Facility: Hillcrest Hospital
Shift: Day/Evening/Night/Weekend
Schedule: 7:00am-3:30pm / 3:00pm-11:30pm
Job Summary
Provides pharmacy services and ensures the appropriate selection, dosing, administration, and dispensing of medication orders.
Responsibilities
- Provide safe, efficient, patient-centered, evidence-based medication services.
- Verify medication orders and/or dispense medications accurately according to state and federal regulations.
- Document services in a complete, accurate and timely manner.
- Actively contribute to medication safety initiatives.
- Ensure all pharmacy operations are in compliance with national accreditation standards.
- Direct staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.
- Educate and train students, residents, other caregivers and/or patients.
Minimum Qualifications
- PharmD or Bachelor of Science in Pharmacy from an accredited college of Pharmacy.
- Current State licensure as a Registered Pharmacist (RPh) or eligibility for reciprocity in the current state within 90 days of hire.
Preferred Qualifications
- Board Certification.
- Completion of a Post‑Graduate Year 1 Residency (PGY1), fellowship or relevant post‑graduate pharmacy experience.
Work Experience
- Completion of a PGY1 residency (fellowship or relevant experience) preferred.
- PGY2 residency or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (e.g., Critical Care, Oncology).
Physical Requirements
- Ability to stand and walk for prolonged times, approximately 75% of shift.
- Manual dexterity sufficient for computer and dispensing functions; ability to lift up to 30 lbs.
- Ability to reach, stretch, stoop, climb steps and sit.
- Must have normal or corrected vision.
- Must be able to communicate verbally by phone or in person.
Personal Protective Equipment
- Follow standard precautions using personal protective equipment as required.
