Responsibilities

• Ensure medications, supplies, and drug information are delivered accurately, promptly, and in full compliance with Joint Commission requirements and other regulatory standards.


• Review and evaluate drug regimens—including complete medication profiles—to identify issues that may require pharmacy intervention or ongoing monitoring.


• Play an active role in direct patient care, collaborating with multidisciplinary teams to guide the optimal use of medications and support coordinated care plans for individual patients and patient groups.


• Provide education to patients and caregivers, build positive relationships with internal and external partners, collaborate effectively with pharmaceutical service representatives, and contribute to quality improvement initiatives across the organization.

Skills & Qualifications

• Experience working in pharmacy with knowledge of treatment modalities and a dedication to patient care.


• Excellent communication skills—tactfully and effectively with patients, staff, physicians, and visitors.


• Ability to provide care appropriate to the age of the patients served in the area.

Minimum Qualifications

• Earned at least a Bachelor's Degree from an accredited pharmacy program.


• Indiana Pharmacist License.


• Cardiopulmonary Resuscitation/Basic Life Support (CPR/BLS) certification.


• Two (2) years of hospital pharmacy experience, preferred.
