Summary

The pharmacist in this position is responsible for all pharmaceutical services in assigned areas. As a member of the multidisciplinary healthcare team the clinical pharmacist promotes an environment of safe, rational, effective and efficient medication use while providing services to meet the needs of the patient. Performs other pharmacy services as appropriate.

Responsibilities

  • Review orders for appropriateness and complete final verification.
  • Assess the effectiveness, safety and affordability of patient medications. Work collaboratively with the healthcare team to develop and optimize patient medication therapy and provide pharmacotherapy consultations as requested in practice area.
  • Provide drug information on medication use, dosage, side effects and composition to physicians, nurses, and patients/families. Monitor inpatient and outpatient profiles for drug interactions, food‑drug interactions, and duplication of therapy.
  • Participate in transitions of care and assist with issues involving patient access to medications.
  • Participate in quality improvement activities such as floor inspections, medication use evaluations, and documentation of interventions. Have knowledge of best practices and how own area integrates with others; demonstrates awareness of the industry, including regulatory requirements, evolving customer demands, and the factors that differentiate the organization in the market.
  • Support contributions to pharmacy and medical literature. Participate in research where applicable. Maintain knowledge in area of expertise. Provide education to pharmacists, pharmacy residents, nurses, and providers. Precept pharmacy students, residents and fellows.
  • Facilitate the compounding, labeling and packaging of medications and pharmaceuticals and provide supervision and verification of technicians work.
  • Respond to code and/or trauma pages when received.
  • Assist in maintaining the department drug inventory at acceptable levels, and ensuring security of narcotics.
  • Participate in the development, implementation and ongoing review of Epic order entry and set build.
  • Attend hospital committee meetings as needed.
  • Participate in continuing education programs, in‑services, and workshops to maintain current knowledge and enhance professional growth.
  • Perform other duties and special projects as required.

Work Experience

  • At least 1 year previous hospital experience preferred.

Education

  • Bachelors Degree Essential Degree (B.S. or Pharm.D) from an accredited school of pharmacy.
  • PhD Degree Preferred Doctorate of Pharmacy and completion of a pediatric clinical pharmacy residency.
  • Required Training/Hours Essential 20 hour ACPE accredited IV training course, except in Retail / First Pharmacy areas. Required within 90 days.

Licenses and Certifications

  • Current Texas Pharmacist license.
  • Basic Life Support. Life Support Course Requirements - Time Frame: 90 Days.
  • Advanced Cardiac Life Support Certification for all emergency care and critical care clinical pharmacists - Time Frame: 180 Days.
  • Pediatric Advanced Life Support Certification for all emergency care and critical care clinical pharmacists - Time Frame: 180 Days.
  • Board of Pharmacy Specialties Certification preferred.
