Job Description

The Clinical Staff Pharmacist provides comprehensive pharmaceutical care to patients and members of the healthcare team to ensure optimal medication therapy outcomes. The Clinical Staff Pharmacist performs medication order evaluation and verification, drug distribution, sterile and extemporaneous compounding, technical supervision, and clinical services. Major job responsibilities include medication order evaluation (to include but not limited to appropriateness and effectiveness) and verification, drug distribution support, technician supervision, clinical intervention, formulary management, and resource for nursing and physician medication related inquiries. The position requires the ability to communicate and collaborate effectively with patients, physicians, nurses, pharmacists, and allied health professionals from all clinical disciplines regarding activities related to the improvement of medication use. Ensures that all activities are consistent with overall hospital objectives. The position demonstrates competence to provide pharmaceutical care for neonatal (birth‑discharge), pediatric (birth to 11 years), adolescent (12‑18), adult (19‑65), and geriatric (66‑over) patients.

Essential Functions

• Deliver comprehensive pharmaceutical care to patients.
• Review medication orders for accuracy, appropriateness and efficacy.
• Discuss medication order clarifications with prescriber, update orders, and notify involved personnel.
• Document all pharmacy interventions in patient electronic medical record and integrated clinical decision support system.
• Monitor drug therapy to evaluate the appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
• Provide pharmacokinetic consultations and other dosing recommendations as required.
• Prevent, detect, monitor, document, and report adverse drug reactions and medication errors.
• Participate in the drug therapy management of medical emergencies such as code blue.
• Perform admission history and discharge medication review and/or counseling to facilitate the admission/discharge/transfer of patients.
• Provide operational support to Clinical Pharmacists and Clinical Pharmacy Specialists as determined by departmental need.
• Respond to drug information requests and optimize pharmacotherapy regimens utilizing current evidence‑based literature.

Promote and Demonstrate Rational, Cost‑Effective Drug Therapy

• Participate in formulary management by converting non‑formulary orders to formulary alternatives when possible; enforce the use of the non‑formulary medication request ordering procedure and validate patient’s home medication when applicable.
• Assist in the procurement of medication when necessary, including verification of home medication supply when applicable.
• Facilitate the implementation of clinical and operational initiatives.
• Recommend and evaluate drug inventory par levels.
• Document clinical consultations and cost avoidance as part of the departmental clinical documentation program.
• Develop formulary monograph or medication use evaluation as assigned by pharmacy leadership.

Mentor Pharmacy Technicians, Students, and Residents

• Provide guidance and supervision to certified pharmacy technicians.
• Precept pharmacy students during IPPE and APPE rotations as necessary.
• Precept PGY1 residents and PGY2 residents as appropriate.
• Participate in scholarly activities including, but not limited to, research, publications, and poster presentations.

Other Functions

• Execute hospital and pharmacy policies and procedures.
• Investigate and resolve automated dispensing cabinet‑related issues.
• Utilize medication carousels to resolve drug distribution‑related issues.
• Participate in orientation of new staff members.
• Complete sterile compounding training, including hazardous medications, and demonstrate competency on a frequency defined by the Sterile Compounding Supervisor.
• Complete extemporaneous compounding training, including hazardous medications, and demonstrate competency on a frequency defined by pharmacy leadership.
• Cover assigned shifts as determined by departmental need.
• Facilitate investigational drug studies with the assistance of the Investigation Drug Service Manager and/or other personnel involved in the drug study.
• Assist in development of policies and procedures as assigned by pharmacy leadership.

Effective Communication Skills

• Utilize advanced communication skills to convey and demonstrate clinical pharmacist expertise.
• Form collaborative relationships with pharmacy team members and other members of the healthcare team.
• Maintain involvement in pharmacy department or hospital committees.
• Achieve and maintain satisfactory working relationships with pharmacy leadership, pharmacy staff and other departments throughout the system, including committees and administration.
• Attend departmental staff meetings.
• Maintain compliance with all company policies, procedures and standards of conduct.
• Comply with HIPAA privacy and security requirements to maintain confidentiality at all times.
• Perform other duties as assigned.

Education

• Graduated from an ACPE accredited College of Pharmacy (required).

Experience

• Post‑Graduation Year 1 (PGY‑1) Pharmacy Residency or three (3) years of relevant work experience (preferred).

Licensure

• Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky (required).

Certification

• Current board certification by Board of Pharmacy Specialties (preferred).
• Current certification in Basic Life Support or must obtain within 90 days from date of hire (required).
• Current certification in Advanced Cardiac Life Support (ACLS) as determined by pharmacy leadership (required).
