SUMMARY:

The ClinicalPharmacistwill be instrumental infacilitatingthe safe and effective use of medication therapy atInterMed.Under the general direction of the Medical Director of Clinical Systems and in collaboration with physician and operational leaders, the ClinicalPharmacistwillparticipatein the development, maintenance, and direction ofInterMed’sclinical programs, including but not exclusively Anticoagulation, Diabetes and Medicated Assisted Therapy (MAT). Critical elements of this position include providing patient, staff, and provider education; performing therapeutic monitoring for efficacy and safety; evaluating for potential drug interaction; making recommendations regardingappropriate dosageadjustment; and effectively communicating to patients, families, and other members of the healthcare team.

CORE RESPONSIBILITIES

• Participate in interdepartmental clinicalquality, value-based careand operational processimprovement and cost savings opportunities, while ensuring compliance with federal and state regulations.
• Remains current on departmental policies, procedures, formularies, and regulatory standards.
• Demonstrates an understanding of the job description, performance expectations, and competency assessment.
• Consultation fororganization’s prescription refill protocol, making recommendations to the Quality Improvement Committee
• Intervenes withappropriate healthcare professionals when necessary, and documents clinical interventions consistent with departmental policy.
• Responsible for understanding andutilizingall technology.
• Maintains courteous and effective interactions with colleagues and patients.
• Demonstrates a commitment toward meeting and exceeding the needs of ourpatients and consistently adheres toInterMedcustomer service standards.
• Pharmacistsare responsible fortheir own professional developmentincluding,meeting all requirements for continuing education, keeping abreast of current professional literature, both from a clinical and medication safety perspective.
• Pharmacists are expected toparticipatein select pharmacy department activities such asthe clinicalstaffmeetingand clinical conferences.
• Performsall other duties as needed or directed by theMedical Director of Clinical Systemsto meet the needs of the Primary Care departments.
• Performs assigned projects in an efficient andtimelymanner.
• Clinical Programs
• Develop, implement, and continually update patient educational materialsfor clinical programs.
• Work closely with all members of the healthcare team including but not exclusive to physicians, NPPAs, nurses, behavioral healthspecialistsand others.
• Develop,maintain, and engage in a collaborative practice model with Primary Care Physicians and NPPAs toinitiate, adjust anddiscontinuemedications, order, and monitor labs and refill prescriptions.
• Provide adequate notes and documentation within the EHR or MTM platform to ensure proper reimbursement, patient safety, and to provide seamless management of patients amongst all staff.
• Clinically reviews anticoagulation medication orders to identify patients receiving therapeutic doses of anticoagulation medications.Assessment should includeappropriate drugselection, medication allergy, dose, route of administration,indicationfor anticoagulation, relevant laboratory results, therapeutic duplication, druginteractionsand age of patient
• Appropriately enters dosing recommendationsand completes bridging calendarsand other pertinent communicationsregardingthe anticoagulation therapy into the Patient’s Medical Record.
• Providesclinical guidance related to anticoagulation therapy to nurses, physicians, interns, medical students, and other members of the health care team.
• Providesanticoagulationdrug information to medical staff and patients upon request or when need isapparent. Documents request into the intervention databasein accordance withdepartmental policy.
• Responsible for day-to-day evaluation of patients on anticoagulation therapy toidentifypatients requiring close monitoring, evaluation of drug interactions,monitoringof therapeutic lab results and communicating recommended adjustments to therapy to thepatient.
• Serves as a liaison between the Anticoagulation Management Service and any health care provider.
• Demonstrates competency in all practice areas in which Anticoagulation Clinical Pharmacy Specialist are performed.Hascompleted all required departmentalcompetency requirements.
• Provides Diabetic drug information to medical staff and patients upon request or when need isapparent. Documents request into the intervention databasein accordance withdepartmental policy.
• Participates in the Pharmacy Residency program as a clinical preceptoras applicable.
• Maintains strict confidentiality in alignment with HIPAA (Health Insurance Portability and Accountability) guidelines andInterMedpolicies.
• Perform other duties to support the mission,visionand values ofInterMed.

KNOWLEDGE, SKILLS, AND ABILITIES

• Education:
• Must be a graduate of an accredited college of pharmacy with current Maine registration in Pharmacy.
• License/Certifications:
• Board Certification in Ambulatory Care Pharmacy
• Experience:
• Strong baseline knowledge of Anticoagulant management.
• Excellent communication skills with a customer-oriented approach.
