BayCare Health Systems is currently seeking an experienced a Clinical Nurse Navigator to join our outstanding and compassionate team at St Joseph's North Hospital in Lutz, Florida ! This position will work within the ECT (Electroconvulsive Therapy) Program.

The Responsibilities Include:

  Serves as the coordinator for the care of patients in their specific specialties.

  n

• n

  Provides support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience.

  n

• n

  Facilitates integrated care through communication with strategic members of the interdisciplinary team, collaborate to provide staff education and work closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.

• n

  Coordinates efforts between physicians, healthcare administration, family, community resources and staff while tracking the progress of the patient throughout their continuum of care.

• n

  Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence.

• n

  Assists in pre-operative, intraoperative and post-operative care of patients in the ECT Program.

  n
n
• n

  Collaborates with ECT Psychiatrist and outpatient providers to consult and initiate ECT treatment.

  Status: Full time, Exempt.

  Schedule: 40 hours per week. Hours may vary.

  Weekend Work: Occasional

Education Required:

n
• Bachelors Degree in Nursing
n

Experience Required:

n
• Two Years’ RN experience in Acute Care or Related Field
n

Preferred Experience:

n
• RN experience in Critical Care or Procedural Care
n

Required Licensure and Certification:

n
• n

  Florida RN License

  n
n
• n

  Certification in Nursing Specialty to related field or obtain within 6 months

  n
n
• n

  ACLS (Advanced Cardiac Life Support)

  n
n
