Responsibilities
- TheClinical Manager will coordinate and oversee all direct care patient services provided by clinical personnel.
- You will conduct assessment and reassessment of patients, update plan of care, evaluate patients' needs and adhere to LeeKO's policies and procedures.
- Monitor and coordinate caregivers' assignments.
- Review referrals for acceptance ensuring patients meet admission criteria.
- Admit patients and assign appropriate clinicians based on the patient's needs.
- Effectively encourages high performance of service delivery and by being available to clinicians during business hours.
- Ensure documentation is in compliance with regulatory agencies and requirements of third‑party payers.
- Ensure final audits/billing are completed timely and in compliance with Medicaid/Medicare and third‑party payers regulations.
- Coordinate communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning.
- Conduct interview and assist with on‑boarding of newly hired clinicians.
- Conduct annual performance evaluation.
- Conduct QAPI and annual CEMP review.
- Perform other related duties as requested by upper management.
Requirements
- Graduate of an accredited School of Nursing.
- Current state or multi‑state license as a Registered Nurse.
- Current CPR.
- Valid driver’s license, with reliable transportation.
- Two years as a Registered Nurse in a home care.
- Preferred at least one year of home health management experience.
