Allaire Health Services is seeking an experienced Hospital/Clinical Liaison to join our growing team in Vermont. This is an excellent opportunity for a motivated healthcare professional with strong relationship-building skills and a passion for driving patient referrals and exceptional care transitions.
Territory Includes:
- Central Vermont
- Additional secondary territories as needed
Position Highlights:
- Full-time opportunity
- Travel throughout the assigned territory is required
- Build and maintain relationships with hospitals, case managers, physicians, and healthcare providers
- Coordinate referrals and facilitate smooth transitions of care
- Represent our skilled nursing and rehabilitation centers with professionalism and integrity
- Work closely with facility leadership to support census growth and community outreach
Qualifications:
- Experience in healthcare marketing, admissions, case management, nursing, or hospital liaison services preferred
- Excellent communication and relationship-building skills
- Self-motivated with the ability to manage a travel-based territory
- Valid driver's license and reliable transportation required
