Clinical Eligibility & Intake Nurse (Fully Remote)

• Redesigning heart health via virtual cardiac rehab for older adults.
• Mission-driven team of clinicians and engineers.

Responsibilities:

• Review EMRs to determine eligibility and create orders.
• Conduct video intake assessments and treatment plans.
• Document, educate, and escalate cases as needed.

Qualifications:

• Active RN or LPN with 3-5 years cardiac or chronic care.
• Epic experience and strong remote communication skills.
