The Role
We are looking for a Clinical Community Pharmacist Manager who can lead both people and process , while expanding our clinical impact.
This is not a traditional pharmacy manager role—you will actively shape care delivery, build programs, and drive engagement across our patient population and partner facilities
What You’ll Own
Workflow & Operational Excellence
- Design and optimize workflow for a ~high-functioning community + LTC hybrid model
- Maximize technician and support staff utilization (top-of-license practice)
- Improve turnaround times, adherence packaging workflows, and delivery coordination
- Bring structure to a fast-moving, service-heavy environment
LTC Consulting & Facility Support
- Serve as consultant pharmacist for partnered SNFs and assisted living facilities
- Conduct and document medication regimen reviews (MRRs)
- Support compliance with Wisconsin LTC regulations
- Collaborate with DONs, administrators, and nursing staff
- Help standardize processes (e.g., controlled substances, packaging differences, med logs)
Remote Patient Monitoring (RPM)
- Oversee RPM programs (BP, weight, and expansion opportunities)
- Monitor clinical alerts and ensure timely interventions
- Engage patients to improve adherence and outcomes
- Support documentation and billing compliance
DSMES & Chronic Care
- Lead or support DSMES programmingIdentify patients for enrollment through pharmacy touchpoints
- Integrate nutrition, medication optimization, and lifestyle coaching
- Collaborate with providers to close care gaps
Clinical Growth & Innovation
- Identify new service lines (CRC screening, opioid harm reduction, preventative care)
- Build scalable systems for patient identification and engagement
- Support integration of CLIA-waived testing and clinical screenings
- Use data and metrics to guide decisions
Team Leadership
- Coach pharmacists and technicians into expanded clinical roles
- Create accountability around workflow, service delivery, and patient engagement
- Support onboarding and training for new services
- Help build a culture that is mission-driven, not task-driven
What We’re Looking For
- PharmD + Wisconsin Pharmacist License (or eligible)
- Experience in community pharmacy, LTC, or ambulatory care
- Strong interest in clinical community pharmacy services + operational leadership
- Ability to move between “big picture strategy” and “day‑to‑day execution”
- Comfortable in a growing, evolving environment (not a static role)
Bonus Points If You Have
- LTC consulting experience
- DSMES or diabetes-focused care experience
- Experience with RPM or chronic care management programs
- Familiarity with Medicare billing for clinical services
- Experience improving workflow or building new programs

- Wisconsin Licensure
- WPQC Certification
- Pharmfurther or equivalent Medical Billing opportunity training
- University of Wisconsin Madison Preceptor training
Package based on a typical 40hrs/week
- $63-72/hr with reviews at 30, 60, 90 days
PSW and NCPA Membership Paid; access to medical billing/FORGE/DSMES training resources through PharmFurther
- Access to training and resources through Flip the Pharmacy and CPESN
- Educational travel/technology/clothing stipend ($5000)
- Meeting registrations mutually agreed upon
