SNI Companies is searching for a Clinical Case Manager to support our client in the downtown area of Jacksonville, FL. Our client is a non-profit organization dedicated to providing medical case management, pharmacy services, health education, and nutrition and mental health counseling for patients and their families. The ideal candidate will have 2+ years of prior Case Management experience and possess a Bachelor's degree.
This is a contract-to-hire, full-time position.
Pay Range: $50K - $60K (depending on education/experience)

Job Summary
The Clinical Case Manager provides case management services which may include: patient intake, assessment of needs, service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment, treatment adherence, case conferencing, crisis intervention, and case closure. Services are provided according to the standards of care developed by the organization/program and may be provided in clients' homes, clinical settings, or a wide variety of community sites.
Responsibilities:

  • Conducts needs assessment of clients with HIV/AIDS in the outpatient and hospital setting for clients to receive comprehensive medical, social and community resources on an ongoing basis or at least every 6 months.
  • Conducts home visits as needed to ensure timely access to and coordination of medical and psychosocial services.
  • Conducts psychosocial assessments, developing, implementing and revising individualized plan of care, family support plan and discharge plan on an ongoing basis.
  • Provide linkages to agency, community and government services and monitor referral completion including state insurance continuation programs, ADAP, medication patients assistance programs (PAP), benefits/health coverage programs such as PAC Waiver, SSI, SSDI, Medicaid, Medicare, housing programs, etc.
  • Educates, links and refers clients to follow-up medical care and community resources to address needs in collaboration with other Network Providers.
  • Advocates for client's unmet needs, providing clinical intervention (counseling, support) as needed.
  • Develops therapeutic relationships with clients, maintaining ongoing connection with clients and families during times they are not accessing services.
  • Assure client awareness of all programs and available community services; advocate for accessibility for all services.
  • Is familiar with appropriate sources of referral for mental health, substance abuse, home care, food, transportation and other support services for people living with HIV/AIDS.
  • Provides referral assistance with above in addition to insurance issues, Medicaid, Medicare, and other public assistance services.
  • Attends weekly clinical staffing providing input regarding patient medical case management needs.
  • Attends other divisional meetings, as required, including weekly research meeting, monthly Case Management Co-Op meetings, monthly division meetings, and other meetings as required.
  • Attends educational activities, trainings and events as required, including monthly HIV Grand Rounds, Annual HIV Conferencing, Medical Case Management training and other trainings as required.
  • Actively participates in community awareness and testing events including National HIV/AIDS Awareness, World AIDS Week and other awareness events.
Requirements:
  • 2+ years of previous Case Management experience within an clinical, outpatient setting providing services to pediatric and adult clients
  • Bachelor's degree in related field required
  • HIV/AIDS, hepatitis, and substance abuse prevention or service coordination experience with the target population preferred
  • Strong multi-tasking and organizational skills
  • Demonstrated ability to develop and maintain a productive and positive atmosphere of teamwork among staff
  • Able to work with people from diverse ethnic, cultural, socioeconomic and sexual preference backgrounds
  • Must be able to work on-site
  • Must be able to pass a background check and drug test
