Clinical Care Manager

Title: Clinical Care Manager (RN, LCSW, or LCPC)

Location: Hybrid Role: Community Visits Throughout Illinois + Remote Documentation

Shift: Monday - Friday, 8:00 AM - 5:00 PM

Compensation: $40 - $45/hr

Interview Process: One round virtual interview

Duration: 6-month contract to hire

Must Haves

• Active Illinois RN, LCSW, or LCPC license
• Associate's degree in nursing or master's degree in social work/Mental Health Counseling
• Clinical or behavioral health experience supporting complex populations
• Valid Illinois driver's license
• Reliable transportation
• Ability to travel within assigned territory
• Comfortable conducting home and community visits

• LTSS (Long-Term Services & Supports) experience
• Medicare and Medicaid population experience
• Behavioral health experience
• Community health experience
• Complex care management experience
• CCM (Certified Case Manager) certification
• Health insurance payer experience
• Experience completing member assessments or UCA/MDS assessments

Day-to-Day Responsibilities

• Meet members in their homes and community settings
• Complete assessments and identify care needs
• Develop individualized, member-centered care plans
• Coordinate healthcare services and community resources
• Support members through care transitions and ongoing health needs
• Connect members with social support and community assistance programs
• Promote wellness, preventive care, and improved health outcomes
• Manage a minimum of 6 member visits per week
• Review member safety alerts and follow established safety protocols during visits
• Maintain documentation and collaborate with interdisciplinary care teams

Additional Details

• Member assignments are typically provided several days to one week in advance.
• Visits are scheduled geographically to reduce travel time and improve efficiency.
• All gas and mileage expenses are reimbursed.
• Consultants are responsible for providing their own PPE.
• Additional safety measures may be implemented for higher-risk member visits

Compensation

$40 to $45

Exact compensation may vary based on several factors, including skills, experience, and education.
