Good Samaritan Health Center seeks a Clinic Care Coordinator RN to support whole-person care for underserved patients in our Atlanta clinic. In this mission-driven, faith-based role, you will provide triage, care coordination, and patient education across primary care, dental, behavioral health, and wellness services. The RN will manage chronic disease workflows, coordinate referrals and follow-up, and ensure accurate EHR documentation.
You will collaborate closely with an interdisciplinary team to address medical needs and social determinants of health, promoting health equity and dignified, compassionate care.
