Help at Home also offers strong career path possibilities for those employees who are committed to growing within the organization.Join us today and become a Help at Home Hero!ResponsibilitiesA typical day includes:Building and maintaining relationships with clients, family members, caregivers, case managers, and team members through regular check-ins, trustworthiness, reliability, and follow-throughUtilizing a relationship-focused approach to make thoughtful client/caregiver matchesProviding high-quality training to caregivers on client's PCISP, including coordinating any additional training required for behavior or risk plansCreating and documenting client schedules based on client need, caregiver availability, and PCISP budget/goalsAttending and participating in quarterly meetings to review client PCISP, design and implement support strategies, and collaborating with the client and their team to reach client goalsMaintaining excellent records, including documentation in EMR, writing and submitting quarterly and annual reports, incident reports, and risk plansFocusing on growth by admitting new clients based on regional goalsCreating great days and meaningful moments for our clients through dedicated, relationship-focused careQualificationsWhat You'll Bring:A positive attitude and the desire to work hard, help people, and make lasting relationshipsExcellent verbal and written communication skillsOrganization skills, with a focus on record-keeping and task-prioritizationStrong independent work ethic and problem-solving abilityExperience working with clients with intellectual or developmental disabilities is a plusA personal vehicle that is insured and meets state safety/licensure requirementsA valid driver's licenseClean background checks and pass a drug test.Education and Experience:At least 18 years of age.High school graduate or equivalent preferred; May require higher level of education or certification.Current PPD, or Chest x-ray if applicable.Medicaid, Waiver, or Home Healthcare experience preferred.Other Requirements pursuant to state or local rules as applicable.Management Authority:Conducts performance reviewsTrains other associatesDirects work of other associatesPhysical Requirements:Ability to move, transport, or position: up to 50 pounds; up to 100 poundsAbility to move or traverse about in offices and/or client homes, including ascending and descending stairs.Ability to communicate effectively and clearly with others to exchange information.Travel Requirements:Regular travel on a daily or weekly basis required, even in inclement weatherThe above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
