Client Care CoordinatorThe Client Care Coordinator is responsible for providing exceptional client care and support to our clients and their families. The clients served involve those with Intellectual and/or Developmental Disabilities ("I/DD") and IHC for Seniors and Disabled. This role requires traveling in and around the Florence/Coastal Area, excellent communication and organizational skills, as well as the ability to build and maintain positive relationships with clients and internal team members.Responsibilities:Coordinate and schedule client appointments and visitsEnsure accurate and timely documentation of client information and care plansRespond to client inquiries and concerns in a professional and timely mannerCollaborate with internal team members to ensure seamless client careAssist in the development and implementation of client care strategiesCoordinate with community partners to ensure seamless client careRequirements:High school diploma or equivalentPrior experience in client care, I/DD, IHC or a related field preferredExcellent communication and interpersonal skillsStrong organizational and time management abilitiesAbility to work independently and as part of a teamMust be able to travel in and around all of the Central Oregon areaBenefits:Hourly Wage of $21-$23, paid every two weeksHealth Benefits (Medical, Vision, and Dental)Mileage ReimbursementPaid time offOngoing training and professional development opportunitiesAbout the Company:Almost Family, LLC is a leading provider of in-home healthcare services.
