Rutland Regional Medical Center seeks a CHT Social Worker to support patients across our community-focused hospital. In this role, you will complete psychosocial assessments, coordinate community resources, and address social determinants of health for diverse patient populations. Working within an interdisciplinary Community Health Team, you’ll collaborate with physicians, nurses, and care managers to develop patient-centered care plans, support care transitions, and reduce barriers to accessing services.
You will also provide crisis intervention, brief counseling, and advocacy for patients and families, maintaining accurate documentation in the EHR. RRMC offers a collaborative, learning-oriented culture with opportunities for professional growth and impact.
