Job Description
Purpose and Scope: This position is responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Responsibilities may include delegation of appropriate tasks to direct patient care staff, such as RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) healthcare team, this role participates in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and facility operations.
Principal Duties and Responsibilities
- Perform all functions under supervision and/or in collaboration with clinical educators or other RNs
- Conduct systematic collection and analysis of patient data before, during, and after hemodialysis
- Assess and document patient/family learning needs and provide education
- Collaborate with patient care staff to ensure safe, effective hemodialysis delivery
- Administer medications and document outcomes
- Respond to emergencies
- Act as a resource for the healthcare team
- Train and orient new staff as needed
- Ensure proper lab specimen collection and rescheduling of missed collections
- Document and update nursing assessment and patient care plans
- Promote awareness of transplant and treatment options
- Perform other duties as assigned
Physical Demands and Working Conditions
- Heavy lifting and patient mobility assistance required
- Extended periods of standing and bending
- May lift up to 30 lbs. and move equipment up to 200 lbs. with assistance
- Work may involve exposure to infectious materials
- Environment includes desk work, computer use, and patient/staff interaction
- May involve travel to other sites
- Employees must take Ishihara’s Color Blindness test (accommodations may be made if necessary)
Supervision
Responsible for overseeing Patient Care Technicians/LPNs/LVNs/RNs as a team lead or designated Nurse in Charge after meeting all of the following:
- Completion of required training and orientation
- Minimum of 9 months RN experience
- Minimum of 3 months of chronic/acute hemodialysis experience as an RN
Education and Licensure
- Graduate of an accredited School of Nursing
- Current state licensure
- Current or completed CPR BLS Certification
- Must meet practice requirements for the state of employment
Experience and Required Skills
- 1 year of Nephrology Nursing experience as an RN within the last two years
- Chronic hemodialysis experience required
