Chief Medical Director, Geriatrics & PACE

Habitat Health seeks a Medical Director to lead the PACE center's clinical strategy and ensure high-quality patient care. The role balances direct clinical care with supervision of the IDT and oversight of care across clinic, home, and facility settings.

You will drive clinical quality, safety, and patient experience improvements, while mentoring clinicians and coordinating with the Executive Director to meet program goals and regulatory requirements.
