Certified Registered Nurse Anesthetist (CRNA)

What we need:

• A "clean" applicant o Physicians with large settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges
• The RFA, CV, and 24 months of relevant activity
• A completed application separate from / following the above step
• References on standby to respond immediately _

• Shift Times: 7am-4pm
• Lunch 1 hour • Provider Min # Shifts Required: see above
• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 8 per day
• Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday

Boards: BC only

• Skills/Certs: BLS, ACLS, PALS Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):

• NOT reimbursed:

o Mileage from provider s home to departing airport, as well as from airport to home upon return.

o Ride share (Lyft/Uber/etc.) and/or taxi from provider s home to departing airport, as well as from airport to home upon return.

• WILL be reimbursed:

o Airport parking at provider s departing airport if provider drives themselves to the airport.

o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.

o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic,
