Licenses and Certification: Illinois Occupational Therapy Assistant License (or letter from the state showing license pending and this job code would be 3545) BLS

- Implements Plan of Care
- Measures and communicates progress towards goals
- Documentation is accurate, complete and timely
- Provides supervision to rehab techs and students
Inpatient: Provide weekend and holiday coverage per department guidelines
