The COTA is responsible for assisting the OT to carry out the treatment plan and occupational therapy program for residents within the scope and limitations set forth in the plan of care and Cheshire Home policy by thorough assessment and development of therapeutic intervention appropriate for each resident. Collaborates with other professional disciplines to ensure effective delivery of care to each resident and to obtain desired outcomes for each resident.
- Assists with assessment, treatment and implementation of resident care.
- Carries out observations and assists OT in administering evaluations.
- Observes, records and reports to the appropriate personnel i.e. Director of Rehab, Clinical Coordinator on a weekly basis, any changes in the residents conditions and/or needs and evaluations through verbal reports, clinical notes and progress notes.
- Interacts directly with other members of the Interdisciplinary Team to provide consistency of care by directly consulting with department heads, when appropriate and attend Care Plan meetings when required.
- Constructs adaptive equipment and self-help devices as recommended by OT.
- Responsible for time sheet, paperwork and residents schedules within the physical therapy department and provide copy to nursing.
- Graduate of OTA curriculum approved by the American Medical Association.
- Has passed the certification examination given by the National Board Certification of American Occupational Therapy Association. Received license from the State of NJ.
- Has at least on year of professional experience, under supervision in a rehabilitation setting.
