We're seeking a passionate Certified Diabetes Care and Education Specialist (CDCES), RN to lead diabetes education initiatives, provide expert patient care, and collaborate with multidisciplinary teams to improve outcomes for patients with diabetes.

What You'll Do

• Provide individualized diabetes education and support for patients and families.


• Manage and educate high-risk diabetes patients using evidence-based care protocols.


• Develop, implement, and evaluate diabetes education programs across the clinic.


• Partner with physicians, nurses, and care teams to improve diabetes outcomes.


• Serve as a diabetes care resource for staff education and patient support.


• Coordinate care and function as a case manager for patients with diabetes.


• Ensure compliance with professional, regulatory, and quality standards.


• Support community outreach, patient education materials, and health promotion initiatives.

Qualifications

Required

• Current Registered Nurse (RN) license


• Certified Diabetes Care and Education Specialist (CDCES/CDE)


• 2+ years of clinical, patient education, or program development experience


• Strong communication, collaboration, and organizational skills

Preferred

• Bachelor\'s or Master\'s degree


• Leadership, supervisory, or program management experience
