nPreventive Services
nPreventive services are provided to families who have been impacted in the child welfare arena. A CPS investigation has taken place due to a child being at risk of harm, abuse or neglect. In efforts to avert a child’s removal from the home and/or to reunify the child back into the family environment, preventive services are mandated by Family First legislation to be provided to families in lieu of foster care placement.
nGeneral Duties:

Responsible for providing weekly home-based intervention services and strategies to families identified by Erie County Department of Social Services (ECDSS) to be at risk for child abuse/neglect.

The Case Planner will be responsible for overseeing 12 cases (families) at any given time.
n n - n
Conduct assessments of ECDSS referred clients and evaluate for other needs that may affect the presenting problem.
n n - n
Establish and maintain a therapeutic relationship with the client.
n n - n
Teach strategies to families to mitigate family disruption and assess safety of children throughout the process.
n n - n
Collaborate with ECDSS and Community Partners to insure effective service delivery.
n n - n
Maintain case files, and in general, meet contractual requirements as described.

Document case progress notes in Connections, a NYS electronic case management system.

Specific Duties :

Provide mandated preventive services, outreach, case planning, assessment, counseling, and face to face contact in the home.
n n - n
Participates in case conferences, service plan reviews, staff training, and court reports, on an ongoing basis.
n n - n
Assist clients in developing measurable goals and individual service plans.
n n - n
Monitor client progress and provide direct assistance, as needed, to ensure intervention compliance.

Advocacy with schools, courts, Erie County Department of Social Services (ECDSS), and other community resources and stakeholders.

Will have access to the company laptop and cell phone.

Qualifications
nQualifications:

Minimum BA/BS degree preferred in human services or related field; and at least one year. experience in case management or implementing direct client services to multi-problem families.

Working knowledge of computers and computer software (NYS CONNECTIONS A PLUS).
n n - n
Effective verbal and written communication skills.
n n - n
The ability to carry out assignments without direct supervision.
n n - n
Typing & Computer skills.
n n - n
Spanish speaking a plus
n n - n
Valid Driver’s License, Car Insurance, and daily access to a vehicle.

Ability to work flexible hours.
