We're seeking a dedicated Case Manager to join a collaborative inpatient rehabilitation team where you'll play a critical role in coordinating care, guiding discharge planning, and advocating for patients every step of the way.

This is an opportunity to combine your clinical expertise with care coordination in a supportive environment focused on helping patients regain independence and improve their quality of life.

What You'll Do

    n
  • n

    Coordinate patient care from admission through discharge

    n
  • n
  • n

    Partner with physicians, therapists, nurses, and families to develop individualized care plans

    n
  • n
  • n

    Lead interdisciplinary team conferences and treatment plan discussions

    n
  • n
  • n

    Facilitate discharge planning and connect patients with community resources

    n
  • n
  • n

    Educate patients and families on rehabilitation goals and expectations

    n
  • n
  • n

    Monitor patient progress, insurance requirements, and care outcomes

    n
  • n
  • n

    Ensure timely and accurate documentation and compliance with regulatory standards

    n
  • n
  • n

    Support utilization review and quality improvement initiatives

    n
  • n

Qualifications

n
    n
  • n

    Active RN, Social Work, PT, OT, ST, Rehabilitation Counseling, or other qualifying healthcare license

    n
  • n
  • n

    Associate Degree in Nursing required for RNs (BSN preferred)

    n
  • n
  • n

    Bachelor's degree required for other eligible disciplines

    n
  • n
  • n

    Rehabilitation experience preferred

    n
  • n
  • n

    Eligible to obtain CCM or ACM certification within two years of hire

    n
  • n
