Position Summary:
The Individual Home Supports Case Manager is responsible for developing, coordinating, and monitoring individualized supports for adults with developmental disabilities who live independently or in community-based settings. This role provides skill training, supportive counseling, case management, and crisis intervention while promoting personal choice, independence, and community inclusion. The Case Manager ensures all services align with person-centered planning principles, best practices, and applicable regulatory standards.

Key Responsibilities:
- Provide direct skill training, case management, supportive counseling, and crisis intervention as needed.
- Facilitate opportunities for individuals to increase independence in areas such as decision-making, community membership, daily living skills, relationships, and personal accomplishments.
- Conduct regular home and community visits to assess safety, well-being, and satisfaction of individuals served.
- Participate in the planning, development, and review of Individual Support Plans (ISPs) to ensure appropriate supports in health, safety, and personal goals.
- Coordinate access to community services, resources, and supports to promote inclusion and independence.
- Provide transportation to appointments, activities, and meetings as needed.
- Collaborate with healthcare providers, community agencies, and other stakeholders to ensure coordinated and effective services.
- Maintain accurate and timely documentation of all services and activities in compliance with agency policies, funding sources, and regulatory requirements.
- Support individuals in developing and maintaining social connections, employment opportunities, and recreational activities.
- Attend team meetings, trainings, and professional development activities.
- Perform other job-related responsibilities as assigned.

- Minimum of 1 years' experience in case management, direct support, or a related human services role.
- Knowledge of person-centered planning, developmental disabilities, and community resources.
- Strong organizational, documentation, and communication skills.
- Ability to work independently while maintaining collaborative relationships with teams, caregivers, and community partners.
- CPR/First Aid Certified (or ability to obtain certification within designated timeframe).
- Valid driver's license, reliable transportation, and willingness to travel extensively.
- Experience in Shared Living, Adult Foster Care, or home-based support models.
- Experience supporting individuals living independently or in community-based settings.
- Familiarity with Medicaid, MassHealth, and related funding sources.
- Knowledge of Electronic Health Record (EHR) systems.
- Bilingual or multilingual abilities.
