THE ROLE

Win is seeking a dedicated and compassionate Case Manager to support families residing in our Transitional Housing programs. In this role, you will provide comprehensive case management services that help families overcome barriers, achieve housing stability, increase income, and move toward self-sufficiency. Through advocacy, coaching, crisis intervention, and collaboration with community partners, you will help clients create and achieve meaningful goals while ensuring compliance with program requirements.

WHAT YOU WILL DO

  • Conduct comprehensive assessments and complete intake documentation for newly arrived families within required timeframes
  • Develop and maintain Individualized Living Plans (ILPs) that support housing, employment, income, and family stability goals
  • Provide case management, crisis intervention, counseling, and supportive services tailored to each family's needs
  • Connect clients to public benefits, healthcare, mental health services, educational opportunities, legal assistance, and community resources
  • Utilize Evidence-Based Practices, Motivational Interviewing, Trauma-Informed Care, and strength-based approaches to support client success
  • Maintain accurate client records, CARES documentation, and program compliance in accordance with DHS, OTDA, and Win standards
  • Collaborate with Housing Coordinators, Income Building staff, Childcare teams, and community partners to advance client goals
  • Monitor client progress, conduct follow-up activities, facilitate referrals, and support families on their path to permanent housing

WHO YOU WILL BE

You will hold a bachelor's degree in social work or a related field with two years of relevant experience, or an associate's degree with three years of client-facing experience

You will have experience supporting individuals and families experiencing homelessness, domestic violence, or other complex life challenges

You will demonstrate knowledge of strength-based case management, Motivational Interviewing, Trauma-Informed Care, and other client-centered approaches

You will be familiar with public benefits, entitlement systems, housing resources, and community-based services

You will possess strong organizational, communication, and documentation skills

You will be comfortable managing a caseload while balancing multiple priorities in a fast-paced environment
