The Case Management Supervisor (reporting to the Director of Social Services) has responsibility for the following:
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Directly supervise the Case Managers

Provide orientation and initial training to new Case Managers, Social Work interns and other social services staff as assigned.

Review daily case notes in CARES and ensure that Case Managers input case notes within 24 hours

Monitor incident reports and ensure that follow up case notes are being attached

Approve documentation in all charts

Identify and coordinate the training and educational needs of the Case Management staff

Facilitate groups that will address the needs of the clients

Provide support to clients who are struggling to follow up on their ILPs

Cover caseloads when necessary

Document client engagement in CARES
-
Audit the clients’ charts to ensure compliance

Participate in case conferences, team meetings, staff training, community meetings, and staff meetings

Approve staff time cards
-
Conduct employee evaluation
-
Perform other duties as assigned.
JOB QUALIFICATIONS:
Educational Requirements:

Master’s Degree in Social Work (Licensed Master Social Worker) or other related fields
Experience:
-
Minimum of 3 years of clinical experience
-
Experience working with the mentally ill and/or homeless population
-
Flexible and comfortable in nontraditional setting
-
Excellent oral and written communication skills
-
Proven supervisory skills (in a supervisory capacity for a minimum of 3 years)
