Responsibilities:
- Oversees staff and program operations for the NJ Housing Supports Program while maintaining a caseload.
- This position ensures quality case management services, program compliance, and adherence to agency, Medicaid, and funding source requirements.
- Supervise, train, and support case management staff through regular individual and team meetings.
- Oversee program operations and ensure compliance with agency standards, Medicaid, and funding source expectations.
- Maintain a caseload, providing assessments, service planning, crisis intervention, housing assistance, referrals, and client advocacy.
- Assist clients with housing applications, provide transportation, and connections to community resources.
- Ensure timely, accurate documentation in HMIS (WellSky) and MyEvolv.
- Submit authorization requests, monitor client eligibility, Medicaid billing, and Medicaid compliance.
- Building and maintaining professional relationships with MCOs. Case conferencing with MCOs. Prepare monthly and quarterly reports and participate in agency and community meetings.
- Bachelor's degree (/BS) required.
- Supervisory and case management experience required.
- Experience working with individuals experiencing homelessness or formerly homeless populations preferred. Valid driver's license with an acceptable driving record. Ability to work independently and work occasional evenings.
