Location: Westfield, MA (serving Westfield, Springfield, and surrounding areas)
Schedule: Monday-Friday, 8:00 AM-4:00 PM or 9:00 AM-5:00 PM
About the Role
Arbor Associates is seeking a dedicated Case Manager to support individuals with developmental disabilities and behavioral health needs across the Westfield, Springfield, and surrounding communities. This is a full-time temporary position covering a leave of absence, with a consistent Monday-Friday daytime schedule.
This role is hands-on and community-based - you'll work directly with clients in their homes and in the community to build independence, stability, and quality of life.

Arbor Associates is seeking a dedicated Case Manager to support individuals with developmental disabilities and behavioral health needs across the Westfield, Springfield, and surrounding communities. This role is hands-on and community-based - you'll work directly with clients in their homes and in the community to build independence, stability, and quality of life.
What You'll Do
- Support clients in developing and maintaining daily living skills
- Assist with housing stability and household management
- Coordinate and reinforce medical appointments and medication routines
- Provide budgeting support and financial literacy coaching
- Facilitate community integration and social skill development
- Transport clients to appointments and community activities using your personal vehicle
- Document progress and maintain case records in accordance with program requirements
What We're Looking For
- Experience working with individuals with disabilities, behavioral health needs, or in a human services setting (preferred)
- Reliable personal vehicle, valid driver's license, and current auto insurance - you will transport clients in your vehicle as part of this role
- Comfort and confidence supporting individuals with complex behavioral or developmental needs
- Strong organizational and communication skills
- Ability to work independently in a community-based environment
- Must be comfortable with ADLs (feeding, toileting, & bathing)
- Able to pass a background check
