nPosition Summary
nWe are seeking a compassionate, organized, and proactive Case Manager to join our treatment team. The Case Manager plays a vital role in supporting clients throughout their recovery by coordinating resources, advocating for their needs, and helping remove barriers to treatment success. This position works closely with clinicians, medical providers, and the management team to ensure clients receive comprehensive, individualized support both during treatment and as they transition back into the community.
nKey Responsibilities
nClient Care & Advocacy
nProvide ongoing case management services addressing clients' housing, transportation, employment, education, legal matters, insurance, benefits, and other psychosocial needs.
nAdvocate for clients' needs within the treatment program and with community agencies.
nAssist clients with applications for community resources, benefits, financial assistance, and supportive services.
nHelp clients complete forms, applications, phone calls, and other tasks that promote independence and life skills.
nCoordinate client appointments with outside providers and community resources as needed.
nFacilitate a rotating Life Skills Group.
nCare Coordination
nCollaborate with clinicians, medical staff, and the management team to communicate client needs, progress, concerns, and barriers to treatment.
nAttend interdisciplinary staff meetings 2–3 times per week.
nAssist with PHQ-9 screenings as needed.
nParticipate in treatment planning and care coordination to support positive client outcomes.
nCollaborate with primary clinicians to complete comprehensive discharge planning and discharge packets to promote long-term success following treatment.
nInsurance Coordination
nPerform monthly verification of all client insurance coverage, including commercial insurance and Medicaid, for both Outpatient (OP) and Residential Treatment Center (RTC) programs.
nIdentify and resolve insurance issues in a timely manner.
nMeet with clients experiencing insurance concerns to review coverage, facilitate calls with insurance companies, and assist in resolving barriers.
nDocument all insurance-related interactions in the client's record.
nMaintain and update the Insurance Issues tracking spreadsheet with accurate and current information.
nDocumentation
nComplete all required documentation in a timely and accurate manner, including:
nCase management notes
nDischarge planning documentation
nEnsure documentation complies with organizational, regulatory, and payer requirements.
nQualifications
nPrevious case management or behavioral health experience preferred.
nKnowledge of community resources, insurance benefits, and social service systems.
nStrong organizational and time management skills.
nExcellent written and verbal communication skills.
nAbility to work collaboratively within a multidisciplinary team.
nStrong problem-solving and advocacy skills.
nAbility to maintain confidentiality and professional boundaries.
