Case Manager, Social Work - Inpatient

Kaiser Permanente seeks an Inpatient Social Work Case Manager to support children and families at our Washington, DC facility. You will conduct psychosocial assessments, coordinate discharge planning, and connect families with community resources while collaborating closely with physicians, nurses, and allied staff.
